Sliding Fee Scale

Insurance

We do not process insurance, but you can contact your insurance company and explore what benefits are available with your plan, and how to submit your own claims.

Payment

We accept cash, check, HSA, FSA, and all major credit and debit cards.

Cancellation Policy

If you fail to keep a scheduled appointment, and have not notified us at least 24 hours in advance, you may be required to pay a portion of your hourly rate.
